Pain in and around the TMJ is referred to as Temporomandibular Disorder (TMD). Most episodes of Temporomandibular Disorders are temporary and resolve on their own with some home care (e.g. heat/ice, soft diet, exercises), but others can last for months to years. When TMDs last more than 6 months, they are considered chronic, and are often accompanied by other pain conditions at the same time.

Research shows that as many as 75% of people with chronic TMD have chronic overlapping pain conditions (COPCs), which contribute to a decreased quality of life and increased healthcare costs. It’s not uncommon for people with TMD to have multiple COPCs. In fact, 8% of people with TMD have four or more COPCs.

COPCs most likely to overlap with TMD are:

Migraine (30%) Chronic lower back pain (30%) Irritable bowel syndrome (24%) Fibromyalgia (22%) Vulvodynia (17%) Interstitial cystitis (13%) Endometriosis (8%) Chronic fatigue syndrome (8%)

You may be wondering how all these pain conditions could be linked. One explanation is central sensitization, which is when the pain centers of the brain become oversensitive, causing a person to experience pain from stimuli that should not induce pain. (Things as gentle as brushing the back of a hand with a cotton ball can cause extreme, intense pain.)

For people with acute TMDs, the take home message is to use home self care for a short period of time, but if symptoms persist, seek conservative treatment from a qualified healthcare provider to prevent the condition from becoming chronic. Emphasis is on conservative treatment, steering clear of anything that is irreversible, to avoid going down a slippery slope.
